Yorkshire Health Partners works with 23 member GP practices spanning across the seven Primary Care Networks in the East Riding PLACE and Hull to support independent partner led primary care. 

We work collaboratively with our member practices to deliver high quality services that achieve and demonstrate the best outcomes and provide an excellent experience for patients.

We have 95% membership out of GP partner led practices for the East Riding area with a combined patient list size in excess of 310,500 patients.
