About Us
Yorkshire Health Partners (YHP) is an integrated community and primary care provider, established in 2012, as a federation of 23 member GP Practices. YHP was initially established to support independent partner led primary care, care closer to home and hold contracts for appropriate schemes for primary care.
YHP’s organisational base is South Cave but we deliver services in collaboration with practices over a number of sites across the East Riding of Yorkshire and Hull.
From 1st October 2026, YHP is responsible for the operational management and delivery of primary care services at Market Weighton GP Practice.
We deliver a variety of NHS services: Children & Young People’s Social Prescribing, Acute Respiratory Infection HUB, Enhanced Access, Dermatology Service, Long Acting Reversible Contraception, Community Diagnostic Centre, Foster & Adoption Medicals, Winter Access Funding, PCN Educator and Independent Prescribers..
We employ over 50 members of dedicated staff who work in, or across, our services and local PCNs and several self-employed clinical staff to provide workforce solutions. Our workforce comprises several disciplines, and includes GP’s, Consultants, Advanced Nurse Practitioners, First Contact Musculoskeletal Practitioners, Nursing, Allied Health Professionals, Pharmacists and administration and clerical staff.
The YHP Board structure consists of 5 Directors, a Non-Executive Director and a Company Secretary.
Primary and community-based care remain engrained into the ethos of YHP and we know the federation can be the bridge between both of these areas.
YHP’s work on Trauma Informed Care
YHP will work collaboratively to ensure that all professionals both clinical and administrative working across the system with vulnerable people who have experienced trauma, can be supported to respond appropriately, consistently and compassionately, so that the support these people receive helps them to thrive.
YHP will be working with the ICB leads in Trauma informed care to understand and implement the Trauma Informed Care organisational toolkit.
