About Us
Yorkshire Health Partners (YHP) is an integrated community and primary care provider, established in 2012, as a federation of 23 member GP Practices. YHP was initially established to support independent partner led primary care, care closer to home and hold contracts for appropriate schemes for primary care.
YHP’s organisational base is South Cave but we deliver services in collaboration with practices over a number of sites across the East Riding of Yorkshire and Hull.
We deliver a variety of NHS services: Children & Young People’s Social Prescribing, Acute Respiratory Infection HUB, Enhanced Access, COVID Medicine Delivery Unit, Dermatology Service, Long Acting Reversible Contraception, Community Diagnostic Centre, Foster & Adoption Medicals, Winter Access Funding, People’s Promise Exemplar, PCN Educator, Independent Prescribers and Primary Care Weight Loss Management.
We employ over 50 members of dedicated staff who work in, or across, our services and local PCNs and several self-employed clinical staff to provide workforce solutions. Our workforce comprises several disciplines, and includes GP’s, Consultants, Advanced Nurse Practitioners, First Contact Musculoskeletal Practitioners, Nursing, Allied Health Professionals, Pharmacists and administration and clerical staff.
The YHP Board structure consists of 8 Directors, 2 Non-Executive Director’s and a Company Secretary.
