YHP's service included in the StreetGames Toolkit

YHP’s Children & Young Person’s Social Prescribing Service are proud to be included as part of the self-assessment tools in the StreetGames Youth Social Prescribing Toolkit. The toolkit has been developed collaboratively by StreetGames, the South West Integrated Personalised Care Team and other key partners across the UK. It is designed to equip professionals with strategies tailored for effective youth social prescribing. The team was set up in collaboration with StreetGames and gives credit to the great work that the team does with children and young people.
