Children & Young Person’s Social Prescribing Service - Case Study October 2023

A summary of the young person’s journey:

The young person was referred for anxiety due to bullying. The young person was not accessing school and mood/wellbeing at home was changing too. The young person’s goal is to be able to get back to “normal” life where they can leave the house on their own and be able to go into town with their friends and access school.

How has social prescribing benefitted the young person?

The young person is now in school every day, accessing a special provision. We have worked on thought challenging and setting mini goals which the young person is doing well with. The young person met myself for their appointment by walking halfway from school to meet me and then walked the rest of the way with me, which was the first time they had been anywhere on their own in a year.
